Seeing Your Doctor:
Please provide as much information as possible when requesting an appointment or visit. Ensure you set a suitable reminder and if you are unable to keep your appointment, please let us know, so we can offer the slot to another patient.
We will endeavour to offer 'urgent' GP appointments on the day you ring or visit. It helps greatly if routine appointments can be booked well in advance.
We allow 10 minutes per appointment with a doctor and slightly longer with the nurse. If you have any special requirements or need longer please discuss this with the receptionist.
New Patient Registrations:
If you wish to register, please complete a medical form GMS1 and health questionnaire for each registering person (print pdf file below or ask at reception). Your previous medical records will be applied for through the health authority. New patients over 5yrs are encouraged to book an initial check up following which a particular doctor will be allocated to allow a more personal service. Area covered by the surgery - .

Home Visits:
Please ring before 10:00am for home visits, which are only for patients too ill to come to the surgery - four patients in the surgery can be seen in the time taken to see one at home. Try to give some details of the illness and the urgency to help doctors plan their rounds. Patients may come to the surgery with temparatures and rashes - it will not endanger others and the receptionist can allow you to wait in a separate area.
Patient Records:
The doctors and staff have a legal duty to keep your records safe and confidential. We observe the strict Data Protection Act requirements.
Comments, Suggestions and Complaints
The practice management team are always willing to receive comments and suggestions and help with any problems as they arise. There is also an in-house complaints procedure - please ask the Practice Manager for details and who will be happy to discuss matters confidentially.
Should this not prove satisfactory, complaints can also be made to the North Hampshire Primary Care Trust.
General Medical Services
Our practice has a 'GMS' contract with North Hampshire Primary Care Trust, which is designed to help GP's focus more clearly on the provision of good clinical care.
Zero Tolerance
We support the Government's NHS zero tolerance campaign. We ask patients to treat GP's and staff courteously, without violence or abuse. Should such behaviour occur, appropriate action will be taken which may involve removal from the Practice List.
In Case of Death - what to do
When a death occurs in the family, whether at home or in hospital, there are daunting and unfamiliar taks to be carried out. Firstly, a 'Medical Certificate of Cause of Death' is required from the doctor in attendance at the hospital or GP if the death took place at home. All deaths must be registered by taking the certificate to the Registrar of Births & Deaths in the sub district in which the death occured (Guildford Registrar 01483 782 071).
The Registrar issues a 'Green' certificate for the burial or cremation. This must be handed to the Funeral Director (various in yellow pages - Luff & Ptns Haslemere 01428 643 524), who should also be contacted without delay if the death has, for any reason, been referred to the coroner. The Funeral Director should be contacted at an early stage as provisional arrangements can be made in advance of the registration of death, if required.
The initial choice of burial or cremation is a personal one for the family to make, unless stated in the deceased's will. The Funeral Director can assist with advice as well as making arrangements. |
