Repeat Prescriptions:

For repeat prescriptions, simply:

- On the tear-off slip attached to your current prescription, tick the items you require.

- Indicate your preferred collection point: the surgery, Lloyds pharmacy (consent form must be signed), or posted to your home (stamped address envelope required).

- Post or bring the slip to the surgery (and place it in the box at reception)

- Collect from your preferred location (normally two working days later from the surgery - extra time is required for the pharmacy and postal options).

Without a tear-off slip, requests must be made in writing or by fax (fax: 0844 477 8749 - a phone request cannot be taken).

Repeat Prescriptions

You may email your repeat prescription (for previously items prescribed within the last 6 months) request to the following email address:

lvsrepeatprescription@nhs.net

This is checked daily.  Please include name, address, date of birth and, if known, NHS number.
